Permanent Deadline Change Request Form

Instructions

Use this form if your program would like to submit a request to:

  • Permanently add/remove entry term(s) for a program,
  • Permanently change a program's admission deadlines,
  • Permanently change a program's admission policy (review definitions), and/or
  • Permanently change a program's decision timeline (review definitions)

This form must be submitted by the program's Department Chair or Graduate Advisor with prior approval from the College Dean. All requests will be submitted to The Graduate College for Graduate Dean approval.

For any new terms to be offered, please indicate the admission deadline date for those terms across each applicant population.

Example: If you are wanting to now allow admissions for the spring semester, enter "October 15" for the "Spring (U.S/Domestic)" deadline and "October 1" for the "Spring (International)" deadline.

Please do not submit deadline dates later than The Graduate College's deadlines:
